I, the undersigned, acknowledge and fully understand that participation in the 417 Elite 50 basketball event involves risks of serious injury, including but not limited to sprains, fractures, concussions, and other bodily harm.
I voluntarily assume all risks of participation for myself/my child and hereby release, discharge, and hold harmless 417 Elite 50, The Proving Grounds, its directors, organizers, staff, volunteers, sponsors, Evangel University, and all affiliated representatives from any and all claims, demands, causes of action, or liability for injuries, illness, damages, or losses sustained during or arising out of participation in this event.
Medical Authorization: In the event of injury or illness, I authorize event staff, volunteers, or medical professionals to provide necessary medical treatment for myself/my child. I further agree to be fully responsible for any medical expenses incurred.
